چکیده

Background Interstitial lung disease (ILD) is an extra-articular manifestation of rheumatoid arthritis (RA) associated with increased morbidity and mortality. High-resolution computed tomography (HRCT) the gold standard for ILD diagnosis, but it has high cost risk ionizing radiation. Lung Ultrasound (LUS) a non-invasive, low-cost, non-ionizing easily performed diagnostic tool. Objectives To establish value LUS in patients RA, using chest HRCT as standard. Methods A cross-sectional study was at Hospital Italiano de Buenos Aires, Argentina. Consecutive RA (ACR/EULAR-2010 criteria), who had least 12 months previous to inclusion.Demographic, clinical, laboratory therapeutic data were recorded. Each patient underwent simplified examination (14 bilateral intercostal spaces -LIS-), assessment B-lines (BL) pleural irregularities (PI). Additionally, 6MWT on each patient. For diagnosis ILD, gold-standard, taking threshold extension interstitial involvement ≥10%, scored by expert specialist. ROC curves made test ability findings (BL PI), 6 MWT, discriminate ILD. Results total 104 included, 21,8 % 82.7% (95% CI 74.0-89.4) women, median age 57.5 years (IQR 47.3 -67.2) duration 8.7 3.3-15.8). 96.2% 90.5-98.9) seropositive factor (RF) and/or anti-citrullinated protein antibodies (ACPA), DAS28-ESR 3.19 2.6-4.6). Methotrexate been used some point 94.2% 87.9-97.8), 46.2% 36.3-56.2) under bDMARD or tsDMARD treatment inclusion. Patients older time (64 vs 54 years, p=0.002) higher erythrocyte sedimentation rate (ERS) C-reactive (CRP) (48 29 mm/h 5.9 3 g/L than without In LUS, more BL (median 26 vs. 1, p <0.001), (Figure 1) PI 4, <0.001) ILD.The accuracy was: AUC 0.88 - 95% 0.78-0.93 0.82 0.74-0.89 4). The best cut-off detection 8 7 PI.The performance sensitive MWT (Table 1). Conclusion presence showed adequate good negative prognostic value. Table 1. Standard and pocket-size lung ultrasound devices can detect interstitial lung disease in rheumatoid arthritis patients.

OBJECTIVES Interstitial lung disease (ILD) is a frequent extra-articular manifestation of RA associated with increased mortality. High-resolution CT (HRCT) is used for diagnosis and follow-up, but its accuracy is counterbalanced by high costs and radiological risk. In the presence of ILD, lung US (LUS) detects vertical artefacts called B-lines. The aims of the present study were to evaluate the...

Rheumatoid arthritis-associated interstitial lung disease

Rheumatoid arthritis (RA) is a systemic disease manifest as a symmetric polyarthritis usually in the setting of elevated autoantibodies (rheumatoid factor). This disease affects 1-2% of the world's population, most frequently in the 25-55 year old age group and has a female predominance (2.5:1). Nearly 50% of patients with RA demonstrate some type of extra-articular manifestation of the disease...

Ultrasound in Rheumatologic Interstitial Lung Disease: A Case Report of Nonspecific Interstitial Pneumonia in Rheumatoid Arthritis

According to the American Thoracic Society (ATS)/European Respiratory Society consensus classification, idiopathic interstitial pneumonias (IIPs) include several clinic-radiologic-pathologic entities: idiopathic pulmonary fibrosis (IPF), usual interstitial pneumonia (UIP), nonspecific interstitial pneumonia (NSIP), cryptogenic organizing pneumonia, acute interstitial pneumonia, respiratory bron...

Usual interstitial pneumonia in rheumatoid arthritis-associated interstitial lung disease.

Interstitial lung disease is a common manifestation of rheumatoid arthritis; however, little is known about factors that influence its prognosis. The aim of the present study was to determine whether or not the usual interstitial pneumonia pattern found on high-resolution computed tomography (HRCT) is of prognostic significance in rheumatoid arthritis-associated interstitial lung disease (RA-IL...
